Activity Summary Custom Instructions not Honored

I recently added custom instructions for generating activity summaries but they're not honored by the model.

Here's what was added to Settings -> Time Entries:

DESCRIPTION RULES
Output one Polish fragment, max 12 words. No period, quotes, bullets or second sentence. The reader is the user, scanning a timesheet weeks later.

• Shape: <object>: <activity> (<tools>). With no clear object: <activity> (<tool>).

• Object: the project, file, document or topic from titles, with names verbatim. If a task or project name is provided, reuse its exact wording.

• Activity: a specific noun (projektowanie, analiza, pisanie, porządkowanie, przegląd, obsługa poczty). Never "praca nad" or "korzystanie z" alone.

• One dominant thread. Add a second after ";" only if it took comparable time. Drop the rest.

• Ignore brief interactions, UI noise (badges, "Nowa karta", localhost, site suffixes) and message content.

• Broad workspace names (Mind Garden, Autonomee) only alongside something more specific.

• Tools go last, max 3, only those that carried the work.

• Use only what the evidence shows. No purpose, inference or hedging ("sugeruje", "prawdopodobnie", "w celu").

• Never open with a time of day, "sesja", "Użytkownik" or "Aktywność".

• Apps only, no titles: coarse activity + app, and don't guess the project.

• Scattered, no dominant thread: a short umbrella label, max two items, no tools.

GOOD

• „Kwalifikacja i sprzedaż": analiza ról w procesie sprzedaży (Figma, ChatGPT)

• Fenbro: analiza raportów efektywności za wrzesień 2026

• Porządkowanie zgłoszeń i etykiet w projekcie Autonomee (Linear)

• Obsługa poczty (Spark Mail)

• Poczta i przegląd notatek

• Praca w terminalu (Ghostty)

BAD

• Poranna sesja poświęcona na czyszczenie skrzynki odbiorczej oraz odpowiadanie na bieżącą korespondencję w Spark Mail. Krótkie interakcje z kalendarzem i Obsidian sugerują synchronizację planów dnia.
  (Too long, two sentences, opens with time of day, infers purpose, gives a brief interaction equal weight.)

• Aktywność obejmowała pracę nad projektem Mind Garden w aplikacjach Pi Web oraz Obsidian, a także korzystanie z ChatGPT, Linear i Ghostty.
  (Narration, a list of tools instead of work, only a broad workspace name, no dominant thread.)

• Użytkownik pracował nad identyfikacją ról w procesie sprzedaży przy użyciu ChatGPT oraz projektował elementy w pliku Figma.
  (Starts with "Użytkownik", tools are the subject, and it has two threads joined by "oraz".)

And yet those are summaries generated today:

In short: they are generated the same way they were before adding custom instructions, as if my rules were not appended to the prompt at all.
